When You Need Transportation

We work with the nonprofit organization CTS. They provide services to people who are disabled and who rely on transportation services to get to their medical appointments.

When you need non-emergency transportation services;

  • Call Tufts Health Unify at 855.393.3154 (TTY: 711). We will transfer your call to CTS. Or you can call CTS directly at 833.242.3331.
  • CTS and its team of experts will process your request. They will schedule your transportation services. And they’ll remind you about upcoming trips.

Medicare Diabetes Prevention Program

Medicare Diabetes Prevention Program (MDPP) services are covered for eligible members with no coinsurance, copayment or deductible. Per a CMS update, MDPP services were made available to eligible members beginning April 1, 2018.

What is the MDPP program?

The MDPP program is for people with prediabetes to help prevent the progression to type 2 diabetes. The program provides education on how to eat healthy, manage stress, increase physical activity and develop skills to maintain weight loss and a healthy lifestyle.

Check the Mail for Your New Medicare Card

The Centers for Medicare & Medicaid Services (CMS) is removing social security numbers from all Medicare cards to lower the risk of identity theft. CMS is sending all beneficiaries new cards with new numbers.

CMS will mail your card between April 2018 and April 2019. There is no cost for the new card. Your current Medicare card is valid until you receive your new card. This change will not affect your Medicare or Tufts Health Unify benefits or your Tufts Health Unify member ID card. Please continue to use your current Tufts Health Unify member ID card.

Helpful Information:

Medicare will send the new cards to the address the Social Security Administration (SSA) has on file for you. To verify that SSA has your current address, visit ssa.gov/myaccount.
