  • Access to in-home urgent care, so you can receive treatment for acute care concerns 24 hours a day, 7 days a week, without having to leave home.
  • Support that goes beyond medical care. You can count on your Cityblock care coordinator to help you with everyday things such as getting quick answers from a doctor, groceries or rides to your appointments.

  • Get health support by phone any time you need it with the Cityblock on-call clinician. Simply call 1-833-904-2273 (TTY: 1-800-720-3479)
  • Use our self-service tool, your member portal, that allows you to access your health plan information 24 hours a day, 7 days a week. You can access the portal from your home computer. Or, download the Tufts Health Plan app to access it from your mobile device.
  • Call the Tufts Health One Care Member Services team at 1-855-393-3154 (TTY: 711), seven days a week, from 8 a.m. to 8 p.m.
