Your care coordinator can work with you on your health goals, manage your care team, organize your care with all your providers, help you make and get to appointments, etc. And you’ll get all your prescriptions and services through one plan.

Your care coordinator meets with you in your home or other safe and convenient place, and works with you to develop a care plan to meet your needs and health goals. Your care coordinator collaborates with your providers and your care team to make sure you get the care you need.
You have a care team that helps you get the care, services and support you need when you need them. Your care team can meet with you where you’re most comfortable – whether it’s in your home, in the community, over video or on the phone.
You may be eligible for Tufts Health One Care if you are 21 to 64 years old and have both MassHealth and Medicare. We’d love to have you as a member.
