What is a One Care plan?

A One Care plan, like Tufts Health One Care, is a health plan option that gives you a better, easier way to manage your health. These plans simplify your care by bringing your MassHealth and Medicare benefits together. 

You’ll have one health plan, one insurance card and one person – a care manager – who coordinates your care. Your care team will personalize and simplify your care with all the benefits that you currently receive through MassHealth and Medicare, plus more.

Your benefits

With Tufts Health One Care, you’ll get more benefits and better care at no added cost to you. You’ll pay $0 copayments and get all your prescriptions through one plan. You’ll also be eligible for added services that you don’t get when you have two separate plans. For example, you’ll have more options for: 

  • Behavioral health
  • In-home support services
  • Care management
  • Medical services
  • Non-medical transportation
  • Help with everyday activities and tasks (personal assistance services)
  • Medical equipment including environmental aids and assistive/adaptive technology, supplies and repairs
  • Dental services
  • Community-based services
  • Instant savings card for select over the counter drugs*
  • YMCA membership*

*Flexible benefits are available if identified in your care plan. Please talk to your care coordinator for more information.
 

The Tufts Health One Care difference

With Tufts Health One Care, you’ll get all your MassHealth and Medicare benefits for your physical and behavioral health (mental health and substance use disorders) needs in one plan. You’ll also get exceptional care management through Cityblock. With Cityblock care management, you’ll have the support you need to get the most from your care plan. 

Together with Cityblock, we’ll work to ensure you get the best and most convenient care so you can reach your full health potential. We’ll help you get holistic support, such as addressing nutritional and housing challenges that affect your health and well-being. We can meet with you and help you apply for food stamps, find local food pantries, access emergency housing, etc.

Eligibility 

You may be eligible for Tufts Health One Care if you are 21 to 64 years old and have MassHealth Standard or MassHealth CommonHealth and Medicare. People are ineligible if they have private health insurance from their job or a home and community-based services waiver. 

Tufts Health One Care serves members in the following counties: Barnstable, Bristol, Essex, Hampden, Hampshire, Middlesex, Norfolk, Plymouth, Suffolk, and Worcester.

Have MassHealth but not Medicare?

Call MassHealth at 1-800-841-2900 (TTY: 711), to find out if you qualify for Medicare based on your health needs.

Tufts Health One Care is a Dual Special Needs Plan (D-SNP) health plan that contracts with both Medicare and MassHealth (Medicaid) to provide benefits of both programs to enrollees. Enrollment in the plan depends on the plan’s contract renewal with Medicare.

The List of Covered Drugs and/or pharmacy and provider networks may change from time to time throughout the year. We will send you a notice before we make a change that affects you.

Benefits may change on January 1 of each year.
