Finding a provider

Your Cityblock care team can help you find a provider who feels like the right fit and can see you in a timely manner. You can call your Cityblock care team at 1-833-904-2273 (TTY: 711).

When you need immediate care

If you believe that you need immediate inpatient care, go to the nearest emergency department (ED) for an evaluation. The ED team will then contact us to determine the next steps based on your clinical needs and your coverage.

When you receive inpatient care for your BH needs, your Cityblock team will provide support to you when you leave the hospital. Members of the team will:

  • Reach out to you within two business days of discharge to support your transition of care
  • Meet with you on an ongoing basis to help with your discharge plan
  • Provide bridge therapy, medication-assisted treatment for opiate and alcohol use disorders and psychiatric care, if needed
  • Provide ongoing medical support and home or virtual visits for urgent needs

Call 1-833-904-2273 (TTY: 711) to talk with your Cityblock care team, 24 hours a day, seven days a week.
