How to apply for Tufts Health Together

Our Tufts Health Together MassHealth plans may be a great option if you lost or don’t have a job, if your employer doesn’t offer health insurance or if you work independently.

You must be a MassHealth member to enroll in one of our Tufts Health Together plans. Not a member? You can enroll in MassHealth any time of the year, if you’re eligible.

How to apply

Apply online:
  • Go to mahealthconnector.org
  • Create a new account
  • Complete an online application
  • Submit your completed application to learn about your eligibility
Apply by phone:

Call the Health Connector at 877-623-6765 (TTY: 877-623-7773), Monday through Friday, from 8 am to 6 pm.

Apply by mail:

Download, print and fill out an application form. Mail the completed and signed form to:

You can also fax your application to MassHealth at 617-887-8770. If you fax your application, write your Social Security number on each page.

You may find out right away about your eligibility. Or the state may review your information and send you a letter telling you their decision about your eligibility.

Once the state confirms your eligibility, you can choose your health plan.
