Childbirth, newborn and breastfeeding classes reward

If you are pregnant and attend a childbirth, newborn or breastfeeding class, you are eligible to receive one $25 gift card per year

How to get your class reward:

  • Print an EXTRAS Reward Form (PDF) or call us for one at 888-257-1985 (TTY: 711), Monday through Friday, from 8 a.m. to 5 p.m. We're happy to help.
  • Fill out the form and make a copy of the form and your receipt for yourself.
  • Mail the completed form and your receipt to:

Watch your mail for your $25 supermarket gift card, which should arrive in six to eight weeks.

Please note:

  • You must be a Tufts Health Together member when you attend a new baby care class and when we process your EXTRAS Reward Form.
  • EXTRAS are subject to change.
