Your Pharmacy Authorization and Claims - Important Information

Starting in January, we had some issues with Optum Rx, our vendor who manages coverage for drugs, that may have affected you.

We want you to know that these are now fixed. Your pharmacist should now be able to give you your prescription drug(s) as needed.

Please note: if you were able to get your prescriptions you can ignore this message.

If you paid for drug(s) that are covered, you can check with your pharmacy to see if they can reimburse you. You will need your receipt. If you did not get a refund from your pharmacy, then we will refund you for those costs. Please fill out the the Prescription Reimbursement Request Form and mail it to OptumRX at:

Once we receive your request, it may take up to 30 days for you to receive your payment.
