We have care managers to help you deal with your condition. DM is for members with a history of asthma, diabetes, congestive heart failure or chronic obstructive pulmonary disease (COPD).
How we help
A care manager can work with you and:
- Answer your questions about your condition
- Help you stick to your treatment plan
- Help you stay away from things that make you feel worse
- Help you set healthy goals
- Help you create a plan to reach your health goals
Your care manager can help you take charge of your health, so you’ll feel better and can avoid trips to the hospital.

Care managers also may reach out to you to see if you would like to participate. To opt out, tell your care manager you’re no longer interested in the program.
