Authorization to Disclose Protected Health Information

Used by a parent not on the policy to certify their right to receive a minor member’s information. This form is only needed if there are no other written authorizations on file, such as a Member’s Authorization to disclose PHI, Designated Representative Form or Legal documentation that appoints the parent as the minor’s personal representative.
