How to get your $25 supermarket gift card:
- Fill out the EXTRAS Reward Form, or call us at 888-257-1985 (TTY: 711), Monday through Friday, from 8 a.m. to 5 p.m. We're happy to help.
- Visit your PCP and have him or her fill out and sign the asthma action plan
- Make a copy of the asthma action plan
- Mail the completed form and the asthma action plan to:

Watch your mail for your $25 gift card, which should arrive in six to eight weeks.
Please Note:
- You must be a Tufts Health Together member when you fill out the asthma action plan and when we process your EXTRAS Reward Form.
- EXTRAS are subject to change.
- Members can get one $25 gift card for completing an asthma action plan every 12 months.
