All rehabilitation facilities are automatically in tier 1, which means you’ll receive tier 1 benefits for using CCHC facilities. See the list of CCHC Rehabilitation Service Locations and Services to best meet your recovery needs. 

Please note that these Rehabilitation Facilities do not show up in the Tufts Health Plan Provider Search tool because they are contracted with the Hospital rather than as individual providers.
