Starting January 1, 2026, Tufts Health One Care will be a Dual Eligible Special Needs Plan (HMO D-SNP). Tufts Health One Care will still combine the benefits of your MassHealth (Medicaid) and Medicare coverage through one health plan. But there will be some important changes and new benefits.
This page is your guide for what you need to know. Be sure to read your 2026 Tufts Health One Care member materials for more information.

You will receive a new member ID number and ID card in the mail, so please keep an eye out for it. Keep using your current ID card through December 31, 2025. On January 1, 2026, you can throw away your current member ID card and begin using the new card you receive in October or later if you’re a new Tufts Health One Care member.
 
 
Your new ID card will have a gold bar on the bottom of the card. Your member ID number is also changing for 2026.

You will continue to receive Care Management services through Cityblock Health. Cityblock is your dedicated team of doctors, nurses, mental health advocates and social workers who will work with you, listen, and do whatever it takes to get you the care you need.

We’ve worked hard to build our provider network in 2026 so we can offer Tufts Health One Care members more options for high quality care. Make sure you’re searching the right plan year before you seek care. You can use our provider search tool to find in network providers at any time:
In 2022, the Centers for Medicare and Medicaid Services (CMS) announced the decision to convert all Medicare-Medicaid Plans, like Tufts Health One Care, to Dual Eligible Special Needs Plans. To be eligible for Tufts Health One Care, you still will need to be eligible for MassHealth (Medicaid) and Medicare.
Current Tufts Health One Care members will be given new ID numbers that are effective January 1, 2026. Members will be mailed new member ID cards with their new member ID number in October 2025.
Members will get new benefits in 2026, including an Instant Savings Card for eligible purchases, and enhanced dental and vision benefits. Please see your 2026 Tufts Health One Care Member Handbook for more details.

We are open seven days a week from 8 a.m. to 8 p.m. (Please note: Our hours shift to Monday through Friday, from April 1 through September 30.) If needed, you may print out an image of your ID card from your member portal to use temporarily while you wait for your new card to arrive.

Any active PA existing Tufts Health One Care members have in place will continue in 2026. If you have PA to see an out-of-network provider, make sure your PA is active before you seek care from that provider to ensure that you are not liable for any out-of-pocket costs. When in doubt, ask your care coordinator or call Member Services.
No. Current Tufts Health One Care members are not required to re-enroll with the plan or MassHealth (Medicaid).

When using the search tool, you will see a banner at the top of the search results screen that will indicate which plan year you’re currently viewing.
Your Member Portal helps you manage your health plan online at your convenience.
If you are registered for the Member Portal, your login information will not change under the new plan, and you do not need to sign up again.
If you haven’t already, sign up today! (link opens outside Tufts Health Plan's website). All you need is a few minutes and your Tufts Health Plan member ID card.
Tufts Health One Care is available in Barnstable, Bristol, Essex, Middlesex, Norfolk, Plymouth, Suffolk and Worcester counties. Starting in 2026, Tufts Health One Care will also be available in Hampden and Hampshire counties.
