Tufts Health One Care covers doctor visits, hospital stays, medication, care management services, vision services, etc. You’ll also be eligible for added services that you don’t get when you have two separate plans. For example, you’ll have more options for:

  • Behavioral health
  • In-home support services
  • Care management
  • Medical services
  • Non-medical transportation
  • Help with everyday activities and tasks (personal assistance services)
  • Medical equipment including environmental aids and assistive/adaptive technology, supplies and repairs
  • Dental services
  • Community-based services

Better, simpler care

As a member of Tufts Health One Care, you’ll get care management services at home. Care coordinators can provide you with services that make a real difference in your health and daily life. Clinicians can work with you and your care coordinator to make sure you get the care you need when you need it. Here are highlights of the services you have access to as a member of Tufts Health One Care:

  • You’ll have a care coordinator who will be with you and your caregivers every step of the way on your health journey.
  • You’ll have the support of a care team dedicated to you and your health. Your team members will meet you wherever and whenever works for you – at home, in the community, or by phone or video visit.
  • Your care team will take the time to find out what’s important to you about your health. They’ll work with you to create a personal care plan to improve your health and daily life. Your team will make sure you get the services you need when you need them.
  • Your care team can help you connect to the right provider or program for your long-term behavioral health goals.
  • Your team will help streamline your care and coordinate your services, tests and appointments with your providers.

You’ll have access to in-home urgent care. No more long wait times – you can get treatment for acute care concerns without having to leave home 24 hours a day/7 days a week.

Becoming a member

You may be eligible for Tufts Health One Care if you are 21 to 64 years old and have MassHealth Standard or MassHealth CommonHealth and Medicare. People are ineligible if they have private health insurance from their job or a home and community-based services waiver. Tufts Health One Care serves members in the following counties: Barnstable, Bristol, Essex, Middlesex, Norfolk, Plymouth, Suffolk, and Worcester.

How to enroll

You can choose Tufts Health One Care as your health plan when you enroll in One Care or anytime if you’re already a One Care member. If you have MassHealth and Medicare, you can easily enroll online, by mail or by phone:
