Complete an ADA Assessment Survey

Take advantage of the opportunity to effectively promote your compliance with the Americans with Disabilities Act (ADA)

By completing a short survey, you can help us update your profile in our Find a Doctor, Hospital, or Pharmacy directory. The survey will give our members information on the level of physical, communication, and programmatic accessibility you provide as part of our network.

Your complete, accurate listing will help you stand out as a provider that meets the requirements of patients with special needs.

Who should participate?

All Tufts Health Plan providers can benefit from participation in the survey by ensuring that their listings in our directory are accurate and up to date. Providers for Tufts Health Unify, our Medicare-Medicaid One Care plan for people ages 21 – 64, are required to complete an ADA compliance and accessibility self-assessment survey.

Prospective and current Tufts Health Plan members can search our website to identify and compare providers for certain types of accessibility. The more information we receive from you, the better we can help members with disabilities identify those providers that can address their particular needs.

Even if you have begun (or are planning) changes that will affect patient accessibility, you should still complete the survey. You can update your responses at any time.

Tips on taking the survey

Please complete the survey once for each practice location. You do not need to complete a separate survey for each provider. We expect that office managers and administrators have the information needed to answer the survey questions.

Make sure to check off each One Care plan you are contracted with in the survey. Doing so will allow us to ensure that all relevant plan members have access to information on your ADA-compliant care.
