Here’s how it works:
Complete and return the Ancillary Provider Application to us with your signed contract and the following supporting documents:
- A current, valid license
- Current and valid accreditation information
- Proof of malpractice insurance
- W-9 for payment purposes
- Completed Federally Required Disclosure Form
Our credentialing team will review your application for completeness and current valid licensure, and submit the application to our credentialing committee for review.
After our credentialing committee reviews your application, we will inform you of our decision.
