Behavioral Health Providers
Start the Credentialing and Contracting Application Process
Note: This process does not apply to hospital-based behavioral health clinicians or to nonphysician behavioral health clinicians billing under a Tufts Health Plan contracted supervising psychiatrist.This process applies to individual practitioners who wish to contract with Tufts Health Plan Commercial products.
To join the Tufts Health Plan provider network, follow the steps below:
Step 1: Submit your credentialing application to CAQH ProView. Note: If you have previously completed the CAQH registration process, log in to update any information that has changed since your last attestation and authorize the release of your data to Tufts Health Plan.
Step 2: Once you have completed the online CAQH application, complete the appropriate Ancillary Practitioner Data Form below:
Step 3: Complete the documents listed below:
- Form W-9
- Copy of your property General Liability insurance (i.e., insurance covering the physical facility in which you work) indicating amounts and dates of coverage for each practice location.

Step 4: Send completed documents to email@example.com.
Step 5: We will email a contract to you.
Step 6: Please sign and return the signature page of the contract to us to the above email address.
Step 7: We will complete the credentialing process and notify you of your effective date. Please do not see Tufts Health Plan members until you have received notification from us.
Tufts Health Plan will only process your application once all required materials are received. You will be notified via email if your application is missing information so please review your materials before submitting them to ensure that everything is complete. Once you submit a complete application, you may check the status of your application by emailing firstname.lastname@example.org.
You will be notified when your credentialing is approved and when your effective date as a contracted provider will begin.
