Clinical Programs - Newborn Care
Through the Tufts Health Priority Newborn Care Program, we provide care management services for medically complex newborns in neonatal intensive care units (NICUs) and special care nurseries within our Massachusetts network. Our nurse care managers assist in the coordination of care among health care providers, provide additional support and education to parents, and assist with planning for the infant's transition home after discharge from the hospital. The program is offered to commercial HMO, POS and PPO members at no out-of-pocket cost to the member.
The goal of the Tufts Health Priority Newborn Care Program is to aid in an infant's transition home and minimize readmissions. The program also aims to provide high levels of family and provider satisfaction and a reduction in neonatology practice variations. These goals will be achieved by:
