Opioid Treatment Program Health Home Program provides coordination of care for persons with opioid dependence disorder who have, or are at risk of another chronic condition. It builds linkages with and among behavioral health care providers, primary care, specialty medical providers, and other community and social supports, and enhances coordination of medical and behavioral healthcare.
The OTP HH program is a service provided to community-based members and collateral providers by a team of professional and paraprofessional staff who are experienced in addiction treatment in accordance with an approved treatment plan for the purpose of ensuring the member’s stability, improved medical outcomes and less reliance on more restrictive services, such as the emergency department, inpatient medical-surgical and/or inpatient psychiatric care.
OTP HH(s) provides person-centered care; providing timely inpatient post-discharge follow-up and improving consumer health outcomes by addressing primary medical, specialist and behavioral health care through direct provision or through contractual or collaborative arrangements with appropriate services provider. The OTP HH places emphasis on monitoring of chronic condition and preventative and education services focus on self-care, wellness and recovery.
Who is eligible?
Opioid Dependent Medicaid recipients who are currently receiving or who meet criteria for Medication Assisted Treatment, and have, or are at risk of another chronic physical health condition are eligible for the OTP Health Home.
The Rhode Island Office of Behavioral Health, Developmental Disability of and Hospitals (BHDDH) licenses Opiate Treatment Programs and OTP Health Homes.
