Your Member ID Card
Once you’re enrolled in Tufts Health Unify, you’ll get a member ID card (see below). This card has important information for you and your providers.
Tufts Health Unify Member ID
- Always show your ID card when you go to a health care provider or pharmacy.
- If your ID card is ever damaged, lost or stolen, call us toll-free at 855.393.3154 (TTY: 711) right away. We will send you a new card.
You can also request a new member ID card with your member portal (link opens outside Tufts Health Plan's website), our self-service tool. With your member portal, you can do things like keep track of your medical history as well as:
- Choose or change your primary care provider (PCP)
- Update your contact information
