With your Tufts Health Unify One Care plan, you get access to high-quality health care services, including the following, at no cost to you:

You also get added support to help you manage your health, such as:

  • A dedicated Cityblock care team of doctors, nurses, mental health advocates and social workers who works with you, listens and does whatever it takes to get you the care you need.
  • Access to in-home urgent care, so you can receive 24/7 treatment for acute care concerns without having to leave home.
  • Support that goes beyond medical care. You can count on Cityblock to help you with everyday things such as getting quick answers from a doctor, groceries or rides to your appointments.
  • A network of trusted primary care providers, specialists and others who understand you and your community. We work with providers, specialists, other health care professionals and member services staff who speak multiple languages and come from the communities we serve.

How to get in touch

  • Health support by phone any time you need it with the Cityblock on-call care coordinator. Simply call 1-833-904-2273.
  • A self-service tool, your member portal, that allows you to access your health plan information 24/7. You can access the portal from your home computer. Or download the Tufts Health Plan app to access it from your mobile device.
