Asthma screening

Get a $25 supermarket gift card for filling out an asthma action plan with your primary care provider (PCP).

  • Print an EXTRAS Reward Form or call us for one at 888-257-1985 (TTY: 711), Monday through Friday, from 8 a.m. to 5 p.m. We're happy to help.
  • Visit your PCP and have him or her fill out and sign the asthma action plan
  • FIll out the EXTRAS Reward Form and make a copy for your records
  • Make a copy of the asthma action plan
  • Mail the completed form and the asthma action plan to:

Watch your mail for your $25 gift card, which should arrive in six to eight weeks.

Please Note:

  • You must be a Tufts Health Together member when you fill out the asthma action plan and when we process your EXTRAS Reward Form.
  • EXTRAS are subject to change.
  • Members can get one $25 gift card for completing an asthma action plan every 12 months.
