Complex care medical management
Our complex care medical management program is for members who have long-lasting medical and behavioral health (mental health and/or substance use) conditions.
We coordinate a team of people (like doctors, nurses, nurse practitioners, and care managers) who will visit your home and work with you and your doctors to make sure you get the right care, in the right place, at the right time.
Your complex care medical management team understands the problems that people with special health care needs have. Your team works with you to arrange for care, services, and equipment; teaches you about your condition(s); and designs the best plan of care for your needs.
You might benefit from our complex care medical management if you are dealing with:
- Multiple health conditions
- Intensive-care newborn needs
- Organ transplant(s)
- Severe disability or impairment

Members who wish to enroll may self-refer to the program by calling 888.257.1985. Data based on medical and pharmacy claims and program standards are also used to identify members.
Your network physicians can also directly refer members to the program by calling 888.257.1985. A nurse care manager will contact the member to discuss his or her needs and interest in the program.
