Explanation of Benefits (EOB)

What are EOBs?

An Explanation of Benefits (EOB) is not a bill. It's a report that gives you useful information about how we processed your health care claims. The EOB includes charges your provider submitted to us and what you might still owe your provider.

When will I get an EOB?

We don't send an EOB for all claims – you will receive an EOB only when we process claims that affect your plan's limits, such as deductibles, co-insurance, and out-of-pocket maximums. EOBs show any financial responsibility you may have for services you have received and they help you make sure bills from your providers are accurate.
