Tufts Health Extend Plan Type II

If you are a Tufts Health Extend member with Plan Type II, select if you are:

  • Age 18 or under

  • Age 19 – 20

  • Age 21+

If you are age 18 or under:

  • You may become eligible for MassHealth.
  • If you haven't done so already, go to MAhealthconnector.org today, create an account, apply for health insurance, and enroll in a plan, such as Tufts Health Plan.
    • Apply for and enroll in your new plan now! Take action early so you don't have to worry about your coverage ending in January 2015.
    • You must apply and enroll in a plan by January 23, 2015, to have coverage on February 1, 2015.
  • The state has likely sent or will send you information in the mail to let you know exactly what you need to do and what your options are.
  • Once this change takes place and you are enrolled in a MassHealth health plan, you should receive a new MassHealth ID card and health plan ID card with your new information.

If you are age 19-20, select your income range:

If you are age 19-20 and earn between 133.1% and 150% of the federal poverty level:

  • You may become eligible for MassHealth Standard.
  • If you haven't done so already, go to MAhealthconnector.org today, create an account, apply for health insurance, and enroll in a plan such as Tufts Health Plan.
    • Apply for and enroll in your new plan now! Take action early so you don't have to worry about your coverage ending in January 2015.
    • You must apply and enroll in a plan by January 23, 2015, to have coverage on February 1, 2015.
  • The state has likely sent or will send you information in the mail to let you know exactly what you need to do and what your options are.
  • Once this change takes place and you are enrolled in a MassHealth health plan, you should receive a new MassHealth ID card and health plan ID card with your new information.

If you are age 19-20 and earn between 150.1% and 200% of the federal poverty level:

  • You may become eligible for a lower-cost health plan through the Health Connector called a ConnectorCare plan.
    • ConnectorCare plans are affordable, silver-level Health Connector plans with federal and state subsidies to help you pay for your health insurance costs.
  • If you haven't done so already, go to MAhealthconnector.org today, create an account, apply for health insurance, and enroll in a plan, such as Tufts Health Direct.
    • Apply for and enroll in your new plan now! Take action early so you don't have to worry about your coverage ending in January 2015.
    • You must apply, enroll in a plan, and pay your premium by January 23, 2015, to have coverage on February 1, 2015.
  • If you choose a ConnectorCare plan based on your eligibility, your costs should remain fairly similar to what you pay today.

  • Once you enroll in a plan through the Health Connector, you should receive a health plan ID card with your new information.

If you are age 21+:
