Two Forms To Remember for Tax Time

What you need to know about the MA 1099-HC and 1095-B

Everyone’s favorite time of year is creeping ever closer. And as you prepare for tax season, don’t forget these two important 2020 tax forms related to your health insurance coverage:

Form MA 1099-HC

This form serves as proof of health insurance coverage for Massachusetts residents age 18 and older. It is required for filing Massachusetts state income taxes.

Tufts Health Plan mailed the 1099-HC form to members who reside in Massachusetts or have a Massachusetts-based employer on or before January 31, 2021.

If it’s after January 31, 2021, and you haven’t received Form MA 1099-HC, but believe you should have, plan subscribers can log in to mytuftshealthplan.com to view and print the form for themselves and their covered dependents from this page. If the form is unavailable online, please contact member services at the number on your ID card.

Form 1095-B Form

This form serves as proof of qualified health insurance coverage (referred to as minimum essential coverage). It is required for filing federal income taxes.

Tufts Health Plan will not be mailing copies of Form 1095-B; it is available online only. Plan subscribers can log into their account at mytuftshealthplan.com to view and print Form 1095-B for themselves and their covered dependents. If the form is unavailable online, please contact member services at the number on your member ID card.
