Tufts Health Priority Care program

Tufts Health Priority Care program reaches out to members who may benefit from the help our nurse case managers when living with complex medical conditions such as:

  • Cancer
  • Diabetes with complications
  • High-risk Pregnancy
  • Multiple Sclerosis
  • Newborn complications
  • Pediatric conditions
  • Spinal injuries
  • Transplants
  • Trauma
  • Stroke

If we identify you may benefit from Tufts Health Priority Care, one of our nurse care managers will contact you to discuss your needs and interest in the program. Together, you'll identify and accomplish positive steps to improve your health and quality of life, including how diet, exercise and other health care support can make a difference.

Our network physicians and care managers can also directly refer you to the program.

Tufts Health Priority Care is intended to support the care prescribed by your doctor. After you are screened and enrolled in the program your Primary Care Provider or other treating physician is sent a letter about your participation. Your nurse case manager works collaboratively with your doctor on an ongoing basis so that all goals support your physician's plan of care. The program is available at no cost to members, and participation is always voluntary.
