Care management services

Based on your child’s health, a care manager may reach out to see if you’d like support.

Care managers can help you:

  • Get extra support for your child’s ongoing medical needs and sudden conditions or events
  • Manage your child’s behavioral health (mental health and substance use disorder) needs
  • Apply for food stamps, find emergency housing and get other resources that support your child’s health

Doula services

We know having a baby can be stressful. We offer a doula program to give pregnant teens extra support.

A doula is a non-medical provider who can support your child during pregnancy, during labor and delivery, and after the baby is born. A doula does not take the place of your child’s provider. Visit TuftsHealthPlan.com/TogetherDoula to learn more.

Nutrition coaching
