    More than 20 years ago we pioneered a collaborative model at the debut of our Medicare Advantage plans. “We knew that, if we were going to be effective in caring for seniors, we needed to do it in a different way—in collaboration with physicians,” explained Patty Blake, president of Senior Products, who helped launch Tufts Medicare Preferred HMO. “We began working closely with provider organizations that were committed to improving quality and delivering care at a price people could afford.”

    A Case Study in Insurer/Caregiver Collaborations: Lifespan Hospitals

    In Rhode Island, Tufts Health Plan teamed up with Lifespan Hospitals to introduce a first-of-its-kind tiered network health plan: Lifespan Premier Choice. A program that delivers exceptional quality health care while reducing costs for subscribers. Lower costs haven’t meant fewer choices though: Lifespan Premier Choice offers subscribers their pick of more than 1,800 physicians across Rhode Island, and members are also eligible to use the Tufts Health Plan network of more than 25,000 providers in Rhode Island and Massachusetts.

    What is Community Rowing, Inc.?

    A boathouse on the Charles River, with a mission not unlike our own: to change lives and communities. We first learned of CRI when they brought indoor rowing to our wellness center for the benefit of our employees. This led to collaboration. 

    With our support, CRI was able to field its first ever inclusive racing team. This year, a CRI-trained crew earned a silver medal at the Paralympic Games in Rio, and the program will field its largest inclusion crew at the annual Head of the Charles Regatta.

An advocate to help answer questions, provide comfort, and coordinate resources for suffering individuals and families—all while leveraging the power of Tufts Health Plan’s resources to get members the help they need.

    Supporting mothers-to-be every step of the way

    A “doula” is a traditional resource in many cultures: a pregnancy coach. They are specially trained to provide a new mother and her family non-medical support before, during, and after the birth of her child.

    Someone to answer questions. An ally. In short, exactly what a mother-to-be needs. Especially when dealing with issues like at-risk pregnancy or challenging economic circumstances.

    Tufts Health Plan piloted the Doula By My Side program, an in-home resource for at-risk expecting mothers. Partnering with the Pettaway Pursuit Foundation (a non-profit organization of certified doulas), our goal was straightforward:

    Empower these mothers-to-be with critical information and an experienced advocate to promote healthier pregnancies.

    And it’s working. Full-term, healthy births with fewer C-sections and pre- and post-partum complications. Of the many births since the program’s inception, exactly NONE of them have required the NICU.

    Because confidence and empowerment can go a long way toward changing an at-risk pregnancy into a healthy, joyful one.

    Not every pregnancy is the same. Sometimes a medical condition with the mother or the child can mean possible health complications. Complications we’re now often able to predict and prevent, saving mother and child from the distress of premature delivery or a stay in the neonatal intensive care unit.

    With a high-risk pregnancy, intensive and constant support is the best way to keep both mother and child on track. That’s why a Tufts Health Plan obstetrical nurse care manager is available to members who qualify. Someone to work with providers and members throughout the whole pregnancy, giving support with everything from treatment plans to information on community and other resources the member might not have known about.

  • Healthy Birthday Program

    We provide every qualified member with a Prenatal Care Guidebook that includes crucial information on screening for pre- and post-partum depression. We empower these expectant mothers with extensive education on crucial issues: fetal development, identifying the signs and symptoms of preterm labor, smoking cessation, and coping with prescribed activity changes such as bed rest (during which our nurse managers provide critical emotional support to help relieve fear and loneliness). In short, we work to reduce risks for mother and child and increase the odds of healthy birthdays for years to come.

    Early intervention and care are key to extending quality of life. That’s why, in the Tufts Health Plan and Alzheimer’s Association model, each member, as well as the primary caregiver, is assigned a dementia care consultant—a social worker who collaborates with them in designing an evolving plan, in consultation with the patient’s primary care physician.

    In 2015, the hotel workers of Unite Here Local 26 faced two unacceptable options: getting less health care, or getting smaller paychecks. For the industrious employees who cover the bustling hospitality industry 24/7, that was no kind of choice at all.

    So the union, employers, and Tufts Health Plan sat down together to build a solution to meet the unique needs of Local 26.
