Department Spotlight: Care Management

Care Management is one of the most important ways we take care of our members at Tufts Health Plan. Our care managers are dedicated nurses and other clinicians who work one-on-one with members, helping them manage chronic or complex health conditions and supporting their mental, physical and social health. We hire people to join our care management team not only on the basis of their experience, but on their commitment to making a positive difference in individual people’s lives.



What’s it like working in Care Management at Tufts Health Plan?

  • Meaningful: Whether you work directly with members or support the people who do, you’re doing work that matters: Guiding people toward healthy choices, helping prevent complications, connecting them with resources and being a source of encouragement and support.
  • Varied: Our team members have to be flexible and resourceful, able to think on their feet. A single afternoon might find a care manager tracking down a pharmacy for a hard-to-fill prescription, arranging last-minute transportation to an appointment, and visiting a member struggling with a new diagnosis.
  • Collaborative: Supporting each other is just as important as supporting our members and coming up with solutions is frequently a team effort. We look to each other for expertise and ideas, and although many of us work remotely, we come together regularly for meetings and events.
