Supporting members with serious or chronic conditions

You may have a chronic illness such as kidney disease or heart failure, or perhaps a short-term health condition such as a high-risk pregnancy. Depending on the condition and the member’s plan, our nurses work with members on specific actions to offer the best opportunity for improved health, and provide information about online resources, social services, pharmacy services, and much more.

Our health programs provide you with a sense of security, knowing that in the event you have a serious or chronic illness or condition, you can get support and information on how to access the quality care you need. These programs support adult, pediatric, and pregnant members with preventive health, chronic conditions, and complex medical needs. Programs are designed to encourage a collaborative partnership between our members and their providers, and include the following:
